Personally Identifiable Information

In accordance with Title 74 O.S. §3106.4, the result of House Bill 2618, the Oklahoma Corporation Commission stores the following Personally Identifiable Information types: Social Security numbers; federal employer identification numbers; billing information related to customer complaints; copies of one of the following documents: driver’s licenses, ID cards issued by federal, state or local government agencies or entities, military cards or draft records, Native American tribal documents, birth certificates, United States passports, reports of birth abroad (FS-240), certificate of birth (FS-545) or Certification of Report of Birth (DS1350), certificates of naturalization, certificates of citizenship, United States Citizen Identification Cards, Northern Mariana Identification Cards, statements provided by US consular officer certifying individual is a U.S. citizen, Form I-551 or unexpired temporary I-551 stamp in foreign passport, Form I-94, Form I-766, Form I-571, or order of an immigration judge granting asylum.

This information is stored on the agency’s secured servers or in secured locations if in paper form.

It is OCC’s practice to release agency records, including PII, in compliance with the Open Records Act, unless they are exempted by the Open Records Act, such as records required to be kept confidential by law, privileged, or are subject to a confidentiality or protective order.
