Long Term Care Administrator

The Health Certification Project offers the Oklahoma Long Term Care Administrator Assessments for the Oklahoma State Department of Health.

• Nursing Home Administrator – 8005 
• Certified Assistant Administrator – 8006 
• Adult Day Care Administrator – 8007
• ICF-IID < 16 beds Administrator – 8008
• Residential Care-Assisted Living Administrator – 8009

Testing Fee: $100

Required Documentation

• Testing authorization letter from OSDH OR provisional certificate issued by OSDH

• Valid, current photo identification issued by government entity within the U.S. (e.g., passport, driver's license, school-issued ID)

• Proof of the SSN:
  • Social Security Card
  • ­Letter from Social Security Administration
  • ­W-2
  • ­Official Tax Return
• If unable to provide proof of SSN, a SSN Attestation Form must be completed
