Identity Theft Passport Program


Victims of identity theft may be eligible for an Identity Theft Passport based on Title 22 O.S. Section 18 (9) or 22 O.S. Section 19a. If you are victim of identity theft based on arrests and court records or credit report, please follow the instructions below.

Arrest and Court Records


If you have been charged or arrested or are the subject of an arrest warrant for a crime that was committed by another person who has appropriated or used your name or other identification without your consent or authorization, follow these steps to apply for an Oklahoma Identity Theft Passport:

STEP #1:  File an Identity Theft Report with a Federal, State or Local Law Enforcement Agency.

STEP #2:  Obtain and submit an Order of Expungement under Title 22 O.S. Section 18 (9) or 22 O.S. Section 19a.

STEP #3:  Complete an Oklahoma Identity Theft Passport Application.

STEP #4:  Submit the following to the OSBI...

  • A certified copy of the expungement
  • A completed Oklahoma Identity Theft Passport Application
  • Two color, passport style photographs of the applicant
  • A copy of the applicant's valid Oklahoma driver's license or Oklahoma ID card*

*If the applicant is a minor, a copy of the minor's parent's or legal guardian's valid Oklahoma driver's license or Oklahoma ID card must also be submitted, along with a sworn and notarized affidavit attesting to the fact that the subject is the applicant's parent or legal guardian.

Upon successful completion of the application process, OSBI will issue a certificate and identification card.

Credit Report

STEP #1:  File an Identity Theft Report with a Federal, State or Local Law Enforcement Agency.  Click here to find steps on what to do.

STEP #2:  Complete the Federal Trade Commission (FTC) Identity Theft Affidavit online by clicking here.

STEP #3:  Submit a copy of the completed Identity Theft Report and FTC Identity Theft Affidavit, with supporting documentation, to one or more of the Consumer Credit Reporting Agencies:

  • Equifax:  800-525-6285, P.O. Box 740241, Atlanta, GA 30374-0241
  • Experian:  888-397-3742, P.O. Box 9532, Allen, TX 75013
  • Trans Union:  800-680-7289, P.O. Box 6790, Fullerton, CA 92834-6790

STEP #4:  Complete an Oklahoma Identity Theft Passport Application.

STEP #5:  Submit the following to the OSBI.

  • A copy of the Identity Theft Report Police Report
  • A copy of your completed FTC Identity Theft Affidavit
  • A certified mail delivery receipt from submission of your completed FTC Identity Theft Affidavit to one of the three consumer reporting agencies.
  • A completed Oklahoma Identity Theft Passport Application
  • Two color, passport style photographs of the applicant
  • A copy of the applicant's valid Oklahoma driver's license or Oklahoma ID card*
