98960-98962 coverage changes

Providers: All

Effective March 31, 2026, OHCA will end current coverage for 98960-98962. These codes were set up in the system several years ago to allow additional reimbursement to providers performing asthma and/or diabetes education to members ages 0-20. After March 31, reimbursement for this education will be included in other services, such as E&M visits or dedicated diabetic self-management codes.  

As of April 1, 2026, 98960-98962 will be used to report community health worker (CHW) services. Information about CHW services and reimbursement will be forthcoming.

We are pleased to announce the release of a new optional prior authorization resource form, jointly developed by Aetna Better Health of Oklahoma, Humana Healthy Horizons, Oklahoma Complete Health and the Oklahoma Psychological Association. Providers can access the form on the contracted entity websites.

This form is intended to support you in the prior authorization process. Use is optional and offered as a helpful resource.

Please note: This form applies only to SoonerSelect contracted entity service providers. For SoonerCare fee‑for‑service providers, the authorization process remains unchanged.
