This service addresses the challenge of fragmented patient care by delivering timely alerts whenever care is administered to attributed patients. The daily report ensures swift action and follow-up on all care events, whether they occur within or outside the participant's organization. Care Fragmentation Alerting helps providers identify missed screenings, overdue vaccinations, unmanaged chronic conditions, and more.

Care Gap Reporting is a powerful tool that enables healthcare providers to efficiently identify and address gaps in patient care through advanced data analytics and real-time reporting. By providing up-to-date patient health data based on customized value sets, it helps providers identify missed screenings, overdue vaccinations, and unmanaged chronic conditions.

This service ensures appropriate access to a patient's consolidated health record at the point of care through the Clinical Portal or Single Sign-On. The record includes key information such as allergies, medications, lab results, radiology reports, and other transcribed documents.

The Medicines Viewer is an integrated, intelligent medicines platform that provides a continually updated view of a patient’s prescribed medications. 

Social Drivers of Health (SDOH) encompass factors like income, education, and environment, which significantly impact health outcomes. MyHealth’s SDOH Program addresses these factors by conducting patient screenings, connecting individuals to community services, assisting Medicare/Medicaid beneficiaries to resources, and collaborating with partners to strengthen service capacity. Oklahoma aims to be the first state to implement universal social needs screening and referral.

Subscribed Notifications are encounter notifications such as ED visits, and hospital admissions or discharges. Providers, if subscribed, will receive HL7 Admissions, Discharges and Transfer messages for their attributed patients.

This service allows for eCQM measurement and reporting to help providers meet mandatory reporting criteria in support of the Medicare and Medicaid Promoting Interoperability Programs, the Merit-based Incentive Payment System (MIPS), the Advanced Alternative Payment Models (Advanced APM), Medicaid Patient Centered Medical Home and other state health programs.

Data submitted electronically to OKSHINE is available in real time to Public Health, enabling epidemiologists to quickly identify public health events, trends, and outbreaks, which allows for faster responses and timely public notifications.
