SoonerCare Supplemental
Medicare is a Federal health insurance program for:
- People age 65 and older
- People under 65 with certain disabilities
- People of all ages with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a kidney transplant)
SoonerCare Supplemental is a plan that may help pay for Medicare cost sharing requirements if you have low income and limited resources. It provides medical benefits that supplement those services covered by Medicare.
- Part A - hospital related premium
- Part B - physician related premium
- Part D - prescription drug coverage
- Cost sharing expense, such as deductibles and co-insurance
Qualifying
Qualified Medicare Beneficiary (QMB)
SoonerCare Supplemental may assist with payments for Medicare Part A and Part B premiums, deductibles and coinsurance QMB is entitled to Medicare Part A and has qualifying low income and limited resources.
Specified Low-Income Medicare Beneficiary (SLMB)
SoonerCare Supplemental may assist with payments for Medicare Part B premiums if SLMB is receiving Medicare Part A and has qualifying low income and limited resources.
Qualifying Individuals (QI-1)
SoonerCare Supplemental may assist with payments for Medicare Part B premiums if the individual is entitled to Medicare Part A, has qualifying low income and limited resources and is not otherwise eligible for full-benefit SoonerCare.
Ways to Apply
- Visit your local DHS office and fill out the application.
- Download the Request for Benefits form from the DHS website, complete it and send it in to the local DHS office. Here's the link to DHS's forms website. You can either type in PS-1 or 08MP001E and click GO to get to the form.
