Rural Health Transformation Program Overview
The Rural Health Transformation Program (RHTP) is a five-year grant through the Centers for Medicaid and Medicare Services (CMS) designed to strengthen healthcare access, quality and outcomes in rural communities through strategic investments in healthcare infrastructure, technology, workforce and care delivery systems.
Oklahoma was awarded approximately $223.5 million for the first budget period to support rural healthcare transformation efforts across the state. The Office of the State Coordinator for Health Information Exchange (OSC HIE) was awarded approximately $16 million to expand health information exchange participation, increase adoption of certified electronic health record (EHR) technology, enhance data and analytics capabilities, and support other initiatives that improve healthcare access and coordination for rural providers and the populations they serve.

OKSHINE Rural Health Transformation Program Areas
Health Information Exchange (HIE) Expansion
Purpose
Increase provider participation in Oklahoma's statewide Health Information Exchange (HIE) through the State Designated Entity (SDE) for HIE (MyHealth Access Network) and improve access to clinical information across care settings.
Key Activities
- Subsidize HIE participation costs for eligible rural providers
- Support onboarding, implementation, training and adoption efforts
- Expand interoperability capabilities, including:
- Image exchange
- Pharmacy/dispensed medication data
- Prescription Drug Monitoring Program (PDMP)
- Mortality registry data
- Public health data
- Enhance social determinants of health (SDOH) screening capabilities

Electronic Health Record (EHR) Expansion
Purpose
Support rural healthcare providers seeking to implement, replace or optimize certified electronic health record systems.

- Subsidize EHR subscriptions and costs for eligible rural providers
- Subsidize infrastructure needs for rural eligible providers
- Deliver site-specific technical support for EHR setup, data migration and education
- Provide a peer-to-peer learning portal to foster knowledge sharing and collaboration among EHR/HIE members

Data Analytics Expansion
Purpose
Expand the use of healthcare data to improve population health, performance measurement and strategic decision-making.

- Support population health initiatives
- Enhance screening and risk identification capabilities
- Leverage HIE data to identify utilization and outcome trends
