Provisional Certification Application Checklist

  • Documentation of Industry Certificate, if required
  • Complete a Teacher Certification* background check:
    http://sde.ok.gov/sde/background-check-teaching-certificate

    *Be sure to use the service code when signing up for your background check appointment.
    2B7KS5 - This code is for a Teacher Certification background check.
    2B7KTN – This code is for a DUAL Employment and Certification background check. Check with your school to see if you need a DUAL or just the certification background check.
