Manual Pricing Information

Some genetic testing codes are manually priced. Please refer to the most current version of the OHCA fee schedule to determine whether a code of interest requires manual pricing.

Codes that are manually priced require submission of an attachment. The attachment must include an itemized list and description of services performed.
Claims that are received without an attachment will be automatically denied.
