Community Health Worker Correction 

The proposed State Plan amendment corrects an error from the initial approval of Community Health Worker (CHW) services. CHW services were originally submitted under the Clinic Services authority (42 CFR 440.90), but at CMS’s request, the authority citation was changed to Preventive Services (42 CFR 440.130(c)). However, the State Plan pages continued to reflect the original Clinic Services authority. To align with CMS’s request to consolidate all Clinic Services pages into a single template, the CHW service page will be moved to the correct Preventive Services section. The amendment also includes a soft monthly service limit based on medical necessity, and reduces the experience requirement from 2,000 hours to 1,000 hours to align with state statute.
