OK SPA 21-0022 Terminate Health Homes

Oklahoma Medicaid will terminate the Health Home program through this State Plan Amendment (SPA) on September 30, 2021; however, other care coordination models currently in place will continue to serve the population impacted by the termination of the program. This SPA does not change any current methods and standards for setting payment rates for services.
