You have indicated interest in obtaining a Food Establishment License. To be eligible for a Food Establishment License, you must:

  • Reach out to the local county health department to determine if your establishment is required to submit a plan review prior to completing a license application.
    • If required, submit plans and plan review fee of $425.00.
    • Receive confirmation from local county health department that plans demonstrate compliance with regulatory requirements.
  • Get a classification number from the county health department (all applicants).
  • Submit a correctly completed license application to the appropriate county health department.
  • Submit license application fee online (this is a separate fee from plan review).
  • Demonstrate compliance with regulatory requirements during pre-licensure inspection.
  • All applicants are required to provide an Affidavit of Lawful Presence with their application.

You have expressed interest in acquiring a Food Manufacturing License. To be eligible for a Food Manufacturing Establishment License, you must:

  • Contact Consumer Health Services at CHSLicensing@health.ok.gov to determine whether plan review is required.
    • If required, submit the food establishment plans to CHSLicensing@health.ok.gov and the $425.00 required plan review fee to the health department (this is a separate fee from the license application fee).
    • Receive confirmation from Consumer Health Services that submitted plans demonstrate compliance with regulatory requirements.
  • Upon receipt of notification that plan review is not required or that submitted plans demonstrate compliance with regulatory requirements, submit a correctly completed license application to CHSLicensing@health.ok.gov.
  • Submit the required license application fee online.
  • Demonstrate compliance with regulatory requirements during pre-licensure inspection.
  • All applicants are required to provide an Affidavit of Lawful Presence with their application.

You have indicated interest in obtaining a Temporary Food Establishment License. To be eligible for a Temporary Food Establishment License, you must:

  • Reach out to the local county health department.
  • Submit license fee:
    • Temporary Retail food establishment - $50.00 for the 1st day and $25.00 for each day after with a maximum of 14 days
    • Farmers Market - $50.00 per day with a maximum of 3 days
    • County Fair - $50.00 per day with a maximum of 3 days
  • Demonstrate compliance with regulatory requirements during the licensure inspection.
  • All applicants are required to provide an Affidavit of Lawful Presence with their application.

  • Plan review identifies whether an establishment will be constructed according to current regulatory requirements, if submitted plans are followed. 
  • A plan review is required before construction of a food establishment, conversion of an existing structure for use as a food establishment or operation, remodeling of a food establishment or change of food establishment type and must be completed prior to the licensure application process. 
  • Potential licensees initiate the plan review process through the local county health department. 
  • If a plan review is required, plan review fees are paid to the county health department prior to completion of a plan review.

  • A license application is submitted to the Oklahoma State Department of Health, along with the required application fee. 
  • A pre-licensure inspection may be completed when the following have occurred: 
    • Plan review fees paid
    • Plan review phase completed
    • License application and fee submitted
    • All application requirements have been met
