Diagnostic X-Ray Permit holder
You have indicated interest in obtaining a Diagnostic X-Ray Permit. To be eligible for a Diagnostic X-Ray Permit, you must:
- Submit a correctly completed license to CHSLicensing@health.ok.gov.
- Be the owner or lessee of the system.
- Submit to an inspection as required.
- Calibrate instrument as required.
- Pay the required permit fees:
- Dental and podiatric facilities - $30.00 per tube
- Veterinary facilities - $25.00 per tube
- All other facility types - $95.00 per tube
- Maximum permit fee per facility (all types) - $500.00
