Medical Micropigmentation

You have expressed interest in becoming a Medical Micropigmentation license holder. To be eligible to hold a Medical Micropigmentation license, you must:

• Be at least 21 years old.
• Submit an affidavit of lawful presence.
• Possess a high school diploma or GED.
• Submit to a background check.
• Complete a Department-approved training program.
• Pass both the written and skills examinations.
• Submit a correctly completed license application and the required $500.00 application fee.

You have expressed interest in becoming a Department-approved Medical Micropigmentation Instructor. To be eligible to become a Medical Micropigmentation Instructor, you must:

• Hold a current Oklahoma Medical Micropigmentation license.
• Have at least three years experience completing eye procedures, full lip procedures, and eyebrow procedures.
• Utilize Department-approved, competency-based instruction including both theory and clinical training.
• Submit instructor application and curriculum to CHSLicensing@health.ok.gov.
