Medical Facilities
The Medical Facilities Division licenses and certifies health care facilities in accordance with state and federal laws. This service has responsibility for inspection, licensure and Medicare re-certification of all non-long-term care medical facilities in Oklahoma.
All licensing fees must accompany the completed application form and be mailed to the following address:
