  • Ambulatory Surgical Centers - Oklahoma licensure of Ambulatory Surgical Centers is required by Title 63, Oklahoma Statutes, Sections 2657 et  seq; OAC Chapter 615 (pdf). Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at www.cms.hhs.gov

  • *Clinical Laboratory Improvement Amendments of 1988 (CLIA) - Federal certification of all clinical laboratories required by Public Law 100-578. Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at https://www.cms.gov/Regulations-and-Guidance/Legislation/CLIA/index.html. See additional Informational Brochures about CLIA

  • *Community Mental Health Centers- Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at: www.cms.hhs.gov

  • *Comprehensive Outpatient Rehabilitation Facilities (CORF) - Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at: www.cms.hhs.gov

  • Hospitals (General Medical Surgical Hospitals; Critical Access Hospitals and other Specialized Hospitals: Psychiatric, Rehabilitation)- Oklahoma licensure of hospitals is required by Title 63, Oklahoma Statutes, Sections 1-701 et seq; Oklahoma Administrative Code (OAC) Chapter 667. Oklahoma licensure of Rural Emergency Hospitals is required by OAC Chapter 667 Emergency Rules. Prospective Payment System (PPS-Exclusion), Special Responsibilities of Medicare Hospitals in Emergency Cases (EMTALA), and other Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at www.cms.hhs.gov.
     
  • *Portable X-Ray-Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at www.cms.hhs.gov

  • *Rehabilitation Agencies (PT, OT, SP)-Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at: www.cms.hhs.gov

  • *Rural Health Clinics -Medicare certification information may be accessed from the Centers for Medicare & Medicaid Services website located at www.cms.hhs.gov

  • Tissue and Eye Banks-Anatomical Gifts - State Permit required by Title 63, Oklahoma Statutes, Sections 2201 et seq. OAC Chapter 505
  • Workplace Drug and Alcohol Testing Facilities - State licensure required by Title 40, Oklahoma Statutes, Sections 551 et seq'. OAC Chapter 638 (100k.pdf)

* These facilities do not have licensing requirements for the state of Oklahoma and require Certification by The Center for Medicare & Medicaid Services.

Please include the CLIA number on your request as well as a contact person and their phone number. Please be aware a new certificate will not be issued automatically. If a new certificate is needed, please see below.

for a replacement certificate. Please include the CLIA number as well as the name of a contact person and their phone number.

 Your new CLIA certificate will arrive approximately 2-3 weeks prior to your expiration date.

Please submit your request to change the director on your CLIA certificate on the CMS-116. All 5 pages must be filled out completely and signed. The form can be emailed to CLIA@health.ok.gov or faxed to (405) 900-7559.  If you have any questions, please contact us via email or at (405) 426-8470.
