Personally Identifiable Data

As per 74 O.S. SS 3106.4, the following information addresses OSDH practices related to personally identifiable information.

What personal information does OSDH collect?

The Oklahoma State Department of Health (OSDH) collects personally identifiable data as required for agency programs as well as from clients receiving county services.

How is this information stored?

Personally identifiable data is stored in a variety of ways, both electronic and paper form, and in compliance with HIPAA rules and regulations. OSDH protects all personal information in accordance with State of Oklahoma Information Security policies, procedures, guidelines (as applicable) found at: https://www.ok.gov/cio/documents/InfoSecPPG.pdf.

With whom does OSDH share this personal information?

Sharing of PII is not just by law. We share PHI/ PII with Public Health Researcher, other Public Health Authority at State & Local Level, local partners, service providers, federal partners or regulatory agencies.

The OSDH limits access to personally identifiable data to the minimum necessary staff. OSDH only shares personally identifiable information with others as required or in accordance with the law.

The Oklahoma State Department of Health (OSDH) is a hybrid covered entity under the HIPAA designation and outlines the use and disclosure of Protected Health Information (PHI)/(IIHI) in the OSDH Privacy Notice. The Notice is posted at every County Health Department and on the OSDH website OSDH Privacy Notice.

Memorandums of Agreement and Memorandums of Understanding

As per 74 O.S. SS 3122, Government Transparency Act, the Oklahoma State Department of Health shall publish a report of all Memorandums of Agreement and Memorandums of Understanding within 15 business days of the effective date, which for each one shall include the:

  • Effective date
  • Duration
  • Entities subject to
  • Purpose
  • Constitutional or statutory provisions allowing for the subject addressed
