Hotel and Lodging License Holder

You have expressed interest in obtaining a Lodging Establishment License. To be eligible for a Lodging Establishment License, you must:

  • Be the owner or officer of the legal ownership.
  • Reach out to the local county health department to determine if your establishment is required to submit a plan review prior to completing a license application.
    • If required, submit lodging establishment plans and the $425 plan review fee to local county health department for review (this is a separate fee from the license application fee).
    • Receive confirmation from county health department that submitted plans demonstrate compliance with regulatory requirements.
  • Upon receipt of notification that plan review is not required or submitted plans demonstrate compliance with regulatory requirements, submit a correctly completed license application to CHSLicensing@health.ok.gov.
  • Required application fee:
    • Category A “Hotels and Motels” (Not more than 20 units) - $300.00
    • Category B “Hotels and Motels” (Not more than 100 units) - $350.00
    • Category C “Hotels and Motels” (More than 100 units) - $400.00
  • All applicants are required to provide an Affidavit of Lawful Presence with their application.
