WIDA Assessment Security Checklists
Checklists for those involved in administering WIDA assessments. These are intended to help LEAs avoid testing irregularities.

Medical Exemption Criteria
Criteria for qualification for a medical exemption from the ELPA written in Oklahoma Administrative Code 210:10-13-23. Medical exemptions must be submitted through Accountability Reporting through a Student Assessment ELP DVR. Medical documentation must be uploaded and submitted during the WIDA ACCESS window.

ELP Assessment Domain Exemption Request Form
Beginning with the 2025-2026 school year, the OELP will require this form to be completed and submitted in a Student Assessment ELP DVR. Once submitted, please see the comments in the DVR to verify the OELP’s approval or denial.
