Reinstatements

Policy for Re-entry to Practice / Reinstatement: OSBOE-P001 

Eligibility

If the applicant was previously licensed in Oklahoma and the license has been canceled for a period of less than two (2) years they may submit an application for reinstatement on their licensee portal.

If the applicant was previously licensed in Oklahoma and the license has been canceled for a period of more than two (2) years they will be required to submit a new application on the applicant portal. However, this application can be linked to their previous profile, which includes documentation from their original application.

If the applicant is also seeking re-entry to practice, they will be required to submit the information located in policy OSBOE-P001, linked above.

Application Process

  • Login to the licensee portal and submit the reinstatement application located in the menu with the applicable fee.
  • Concurrently, the physician will need to work on completing the below:
  • Following submittal, the licensing team will review and reach out to request the other items required for reinstatement.
  • Once all items have been received, the licensing team will inform the applicant if an appearance before the Board is required prior to action on their application.

All licenses issued prior to May 1 will require renewal by June 30.

  • Login to the applicant portal and submit the correct application for the license being requested. NOTE: In order to link your prior application/license information reach out to the office to ensure OSBOE has the correct email address on file.  Once complete the applicant will login to the portal by resetting the password.
  • The following documents will be in the applicant's original file:
    • National Examination Scores
    • Medical School Verification and Transcripts
    • Post Graduate Training Verification (if additional training was completed following initial license issuance, applicant will need to request the verification for that training)
  • Follow initial application instructions and begin to gather all other required documentation.
  • The licensing team will update the application with what is still required for the applicant's review.
  • Once all documentation is received, the licensing team will inform the applicant if an appearance before the Board is required.

All licenses issued prior to May 1 will require renewal by June 30.

For physicians that have not held an Oklahoma license and have not practiced medicine for a period of greater than two (2) years, the following will need to be submitted in addition to the application:

  1. Length of time in practice prior to departure, and practice areas prior to departure. 
  2. Detailed explanation of the time out of clinical practice; 
  3. All Continued Medical Education (CME) completed during their absence from clinical practice; 
  4. Administrative, consultative activities, educational, supervisory or mentoring responsibilities during the time out of practice; 
  5. Concise plan for re-entry to practice and detailed description of future scope of practice plans. The plan must include, but is not limited to, the proposed practice site, whether sole practice or in a group, status of hospital privileges, qualifications as an approved provider by any insurance company or third-party payer, malpractice claim history, criminal history, or other litigation history; and
  6. Current status with any specialty board, Oklahoma Bureau of Narcotics and Dangerous Drugs (OBNDD) and Drug Enforcement Administration (DEA) registration, Oklahoma Health Professional Program status, and any academic appointments or medical administrative positions.

Following submittal and processing of above information and reinstatement application, a Board appearance may be required.  Each re-entry request is evaluated on a case-by-case basis, and additional requirements may be requested of the applicant.
