Name Changes

To submit a legal name change the licensee will follow the below process:

  • Login to the licensee portal using previously created credentials and select "Name Change Request" from the available menu.
  • Enter the new legal name as it should appear on the license.
  • Upload proof of the name change (Marriage License, Divorce Decree, Court Order, etc)
  • Once submitted, the licensee will be notified by email once it has been processed or if additional information is required.

If the applicant would like to order a new wall license to accompany this name change, they will select "Document Request" from the menu and select "Duplicate Wall License" and submit that request with the applicable fee.
