Document Requests

The following documents can be requested via the licensee portal by licensees or the form linked below.

Licensee Portal - select "Document Request"

License Request Form - this can be submitted via mail with a check payment or if paying by credit card, a link to a secure payment will be sent to the requestor upon receipt and request via email.

Processing of these requests takes an average of 3-5 business days following receipt of payment.

Typically requested by a different state licensure board and ordered by the licensee, this document will verify the licensees status and disciplinary history, if applicable.

The licensee will provide OSBOE with the receipients information at the time of request.

If the recipient accepts this letter by email, please provide it when requesting for faster processing.

There are two versions of this letter.  One is addressed to the recipient of the licensee/requestors choosing and the other is addressed to the Secretary of State.

The version addressed to the Secretary of State should be included in the package for incorporating.

The version addressed at the choice of the licensee is typically used for credentialing, by employer request, etc..

When requesting online, the licensee will need to verify the option they are choosing is correct for their needs.

Licensure files are considered public records and can be requested by the physician or members of the public.

If requested by the public, all personal information is redacted before the file is provided.

If requested by the physician, the file remains unredacted and is provided at no charge.

A disciplinary file is public and can be requested by the licensee or members of the public.

These are comprised of orders issued by the Board to a physician.

Not every physician has a disciplinary file and if the license file is requested, the disciplinary file will be included as it is part of the record.

A physician can request a duplicate wall license in their licensee portal with payment of the applicable fee.

If this is prompted by a name change, OSBOE asks the physician indicate so in the description so the wall license isn't printed until the name change has processed.

A physician database is public and can be ordered for a number of reasons.  It does not include an private physician data.  

This will be delivered to the requestor as an excel spreadsheet with the following colums:

  • Name
  • Gender
  • License Number
  • Disciplinary Action (Yes/No)
  • License Type
  • License Status
  • Issue Date
  • Expiration Date
  • Specialty / Specialties
  • Board Certified (Yes/No)
  • Practice Name
  • Practice Address
  • Practice Phone
