Application Types
To begin, the applicant will first need to understand which application type they need. Read below for a brief description of each application.
This is the standard licensure application. These are separated into two categories in the system. One for those utilizing the FCVS service and one for those who are not. FCVS is described in more detail on a separate page.
NOTE: Residents entering their second year (PGY2) will utilize this application to be issued an Advanced Resident license. This is a requirement to begin PGY2. No practice shall be conducted without a license issued.
This application for residents entering their first year of residency (PGY1). It is a requirement to begin PGY1. See 510:10-3-11
This application is for out-of-state residents coming to Oklahoma to rotate for a maximum of six (6) months. This is a non-renewable license and does not grant prescriptive abilities. See 510:10-3-12
This application for is active-duty military members who qualify for a temporary license while they are in Oklahoma and choose to moonlight off base or federal property. See 510:10-3-14
This application is for physicians who are retired from active practice and wish to donate their services. See 510:10-3-13
This application is only available to current licensees who are fully retired from the practice of medicine and wish to keep a license with the Board.
This license does not allow for the physician to practice medicine or prescribe medication.
Applicants must have held a full and unrestricted license in Oklahoma for at least ten (10) years. See 510:10-3-13
