License Renewal
All artist licenses and all shop licenses are valid for one (1) year from the date of issue.
OSDH will send a courtesy renewal notice to all license holders one (1) month before the license expiration month. The renewal notice will be sent to the last known mailing address OSDH has on file. To update your mailing information, please fill out the Change of Information form.
- There is a thirty (30) day grace period for license renewals before a late fee is assessed.
The grace period only applies to the late fee; IT DOES NOT PERMIT AN ARTIST TO PRACTICE WITH AN EXPIRED LICENSE OR IN A SHOP WITH AN EXPIRED LICENSE.
NOTE: the artist or establishment is responsible for renewing their license before it expires along with any associated late fees, whether or not they receive a renewal notice.
It is unlawful to perform any procedure without a current license in the appropriate category.
Submit the renewal fee (including late fee if after 30 days of expiration) and current annual trainings, as well as the renewal notice OR a copy of the license by:
