Community Health Improvement Efforts
McClain County Health Improvement Plan
The McClain County Health Department, in partnership with the Blanchard Community Coalition, a Turning Point partner, Community Youth Action Program (C.Y.A.P), Community Alliance of Resources for Everyone (C.A.R.E) and other community partners work to improve the health and well-being of McClain County residents. This effort addresses the current health status of McClain County residents and creates initiatives to strengthen communities, unite vision, reduce health disparities, and unify a community to promote a positive healthy environment to live, work, and play.
The McClain County Health Needs Assessment for 2011 (PDF, 4.4MB) is now available. This assessment gives a snapshot of the heath statistics for McClain County.
The McClain County Health Improvement Plan (PDF, 1.8MB) is now available. This plan addresses the health status of the county, and is a follow-up to the needs assessment.
