Oklahoma J1 and NIW Waivers
The Oklahoma J1 “Conrad 30” Program has reached capacity. No additional Conrad 30 Waivers are available.
National Interest Waiver and Health and Human Services Waivers can be submitted with an application.
Oklahoma is committed to ensuring all residents have access to health care. The Oklahoma State Department of Health is enabled to recommend the approvals of J1 Visa Waivers to the US Department of State J1 Visa Waiver Program and the Health and Human Services Waiver Program. Oklahoma is allocated 30 “Conrad 30” slots for which OSDH submits applications directly to the Department of State; Health and Human Service (HHS) Waivers are unlimited; OSDH providers a letter of support to applicants.
The submission of a complete waiver package to the OSDH does not ensure that the OSDH will recommend a waiver. Applications for waivers are reviewed in the order in which they are received. If the number of applications received exceeds the number of waivers available, the OSDH reserves the right to prioritize applications based on geographic distribution and/or degree of shortage for specific physician specialties. In all instances, OSDH reserves the right to recommend or decline any request for a waiver.
The OSDH Office of Primary Care and Rural Health Development (OKPCO) is responsible for coordinating the J1 waiver application submission and review process. Recommendations to the U.S. Department of State are issued by the Oklahoma Commissioner of Health.

Application Requirements & Instructions
The Oklahoma J1 “Conrad 30” Program has reached capacity. No additional Conrad 30 Waivers are available.
Applicants can submit a signed letter of intent or employment contract - OR- a completed application, Letters of intent or signed employment contracts will tentatively hold an J1 Visa Waiver slot for up to three weeks. After three weeks, approval cannot be guaranteed. Please note, if an applicant is eligible for an HHS Waiver, the application will be returned with a request to submit a new application for an HHS waiver.
Applications will be reviewed and approved in the order in which they are received. If applications are incomplete or require more information, applicants have ten business days to provide corrected documents before slot is released.
For completed applications, please limit documents submitted to those listed below; The case number must be included on all documents included in application. Please assemble applications in the following order:

*Please note that facility name(s) and each address where the applicant will be working must be in the contract.
Please also note that recruitment effort documents should NOT be submitted with Conrad 30 applications. This and/or other additional documentation submitted with an application must be manually removed and may therefore lead to increased application processing times.
HHS waivers are unlimited and available to primary care specialists (Family Medicine, General Practice, OB-Gyn, Pediatrics, and Internal Medicine/Hospitalist). To qualify, physicians must have completed a residency program within the past year and must have a signed employment contract with a facility located in a health professional shortage area with a score above 7.
To request a letter of support for an HHS J1 waiver, applicants should complete the Oklahoma J-1 Visa Waiver Applications 2025-2026 form and attach the following documents:
- Request for letter of support:
- Signed employment contract for at least three years; contract must include facility information and address;
- HPSA/MUA Evidence documents - Find Shortage Areas by Address
The Physician National Interest Waiver is a special provision under U.S. immigration law that allows foreign physicians to bypass the labor certification process (PERM) typically required for employment-based green cards. Instead, physicians can apply directly for a green card if they agree to work full-time in a clinical practice for a five- year period, and in a geographic area that has a shortage of healthcare professionals or serves a medically underserved population.
To request a letter of support and attestation for a NIW, applicants should complete the Oklahoma J-1 Visa Waiver Applications 2025-2026 form and attach the following documents:
- Letter of request for support that includes:
- Physician name and specialty.
- Facility addresses and address of all sites at which physician will practice.
- Explanation of physician status:
- If the physician currently holds a J1 Visa Waiver supported by Oklahoma, please provide details and Department of State Case Number on original application.
- If physician is completing a J1 Service Obligation in another state, please provide a letter from that state confirming the terms of the physician’s J1 service obligation and completion date, if available. Additionally, the Primary Care Office for the state from which the physician is transferring must provide a letter of support for the transfer of service credit.
- Signed employment contract or addendum to contract that indicates the requisite length of obligation; contract must include facility information and address.
- HPSA/MUA Evidence documents - Find Shortage Areas by Address
An application for a J1 Visa Waiver or request for letters of support for HHS and
National Interest Waivers must be submitted by a health care facility or an attorney representing the physician and/or the facility.
- Conrad 30 Waiver applications can be submitted any time after September 15,
each year. Applications will be reviewed on a weekly basis. Typical processing time for approval and recommendation of complete applications is three weeks. - Request for letters of support for HHS and National Interest Waivers may be
submitted at any time.
