Reports and Billing
External Clients
Test reports are issued to external facilities and health care providers via secure email, fax and/or U.S. mail using the contact information provided on the test requisition form or as previously provided to and verified by the OSDH PHL. Facilities/providers must complete an Email Verification Form or a Facsimile Verification Form prior to receiving reports by email or fax. To enroll in electronic reporting option, contact the PHL at (405) 564-7750.
County Health Departments
OSDH County Health Departments may retrieve patient test reports directly through the OSDH Public Health Oklahoma Client Information System (PHOCIS) or Patagonia Electronic Health Record.
Newborn Screening Results
Newborn screening results are available to authorized health care providers via the Newborn Screening Results Web Portal. For new user access or questions regarding the NBS results portal, please contact the PHL Client Services Department at (405) 564-7750 or PublicHealthlab@health.ok.gov.
Certain critical NBS test results may be communicated by the OSDH Newborn Screening follow-up team to authorized facilities or health care providers via telephone. All results reported by telephone require read-back confirmation by the recipient.
The OSDH PHL may issue corrected reports when necessary after results have been released. Corrected reports clearly identify the information being changed (including “from” and “to” entries) and the reason for the correction, when applicable. The submitting facility will be asked to return or securely destroy the original report.
Changes to test report information may be requested by an authorized representative of the submitting facility. Please call the OSDH PHL (405) 564-7750 and ask to speak to the supervisor of the laboratory section responsible for performing the test. Verbal requests must be followed by a written request within 30 days. No corrected report will be issued until written documentation is received. Required forms to document the change will be provided by the laboratory supervisor or OSDH PHL Client Services Department.
Requests to correct demographic information after report issuance will result in a corrected report that documents the specific change (with explicit indication of “from” and “to”), the individual requesting the correction, and the date and time of the request. A request will be made to the submitting facility for the original report to be returned to the OSDH PHL or for it to be destroyed by the submitting facility.
The OSDH PHL may correct minor typographical errors in patient names; however, patient identity will not be changed unless the original requisition contained incorrect information, specimen labeling was misinterpreted during data entry, or a data entry error occurred.
On February 6, 2014, the Centers for Medicare & Medicaid Services (CMS) issued a final rule amending CLIA and HIPAA to provide patients with direct access to their laboratory test results. Under this rule, HIPAA-covered laboratories must provide patients or their authorized representatives with results upon request.
The OSDH PHL in Stillwater does not release test results directly to individuals at the laboratory. Patients or legal representatives may obtain copies of test reports through the County Health Department or health care facility where care was provided. Individuals will be required to complete an ODH 206 Oklahoma Standard Authorization and present valid photo identification and/or an authorization code prior to release.
Alternatively, patients or legal representatives may contact the OSDH PHL at (405) 564-7750 or via email at PublicHealthlab@health.ok.gov to request laboratory test reports by mail or electronically. Requests must be submitted using the Request to Release Lab Test Results form.
CPT codes provided by the OSDH PHL are for reference purposes only and reflect the laboratory’s interpretation of applicable coding guidelines. CPT coding and billing are the responsibility of the submitting or billing entity. Facilities should contact the OSDH PHL for information regarding testing methodology and/or consult their local Medicare carrier for coding clarification, as needed.
