Located on this page is the Request for Proposal (RFP) for National Initiative to Address COVID-19 Public Health Disparities among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities.

The Contract is awarded on behalf of the Oklahoma State Dept of Health (OSDH) to establish an Oklahoma partnership to address health disparity and equity initiative. The Oklahoma State Department of Health is making funding available to address COVID-19 related disparities under the recently awarded CDC COVID-19 disparities funding (CDC-RFA-OT21-2103). A total of $3,000,000 in mini-grants will be awarded to Oklahoma federally recognized tribal nations. Individual award amounts will vary depending on the proposed activities, proposed budget, potential impact, and number of applications received, with a cap of $300,000.

  • All questions pertaining to this solicitation should be emailed to donnad@health.ok.gov before 3:00 p.m., November 1, 2021.
  • Request for administrative review by OSDH no later than 3:00 p.m. on November 1, 2021, to be considered for award.
  • Bid response to OSDH no later than 3:00 p.m. on November 30, 2021, to be considered for award.
