Required Documents

Before You Begin

Depending on the service that you are looking to complete, you will be required to provide acceptable documents. Currently, there are two categories of documents: Primary ID and Proof of Oklahoma Residency. Acceptable documents for each are listed below.

Not every service will require documents from each category

Refer to the specific service page to understand what documents are required

Photo copies of your documents are not accepted

Getting a Real ID?

Use the Real ID checklist to see the required documents needed to add a REAL ID credential to your License or ID card.

Primary ID Documents

You will need to provide proof of identity/lawful presence in the U.S. The document must be an original or a certified copy. Notarized documents are not accepted. Examples of proof of identity include:

  • Birth Certificate issued by the state in which you were born
  • Consular Report of Birth Abroad
  • Unexpired U.S. Passport
  • Unexpired Permanent Resident Card
  • Naturalization/Citizenship Certificate
  • Unexpired Employment Authorization Card
  • Valid, unexpired foreign passport with a valid U.S. visa and valid I-94 issued by DHS. Other USCIS documents may be required depending on your status (DS-2019, I-20, etc.).

If your name is different from the document which is presented from above (due to adoption, marriage, divorce, court-ordered name change), or is not the same on all your documents, you must provide additional documents with legal proof of every name change.

Each document must be an original or a certified copy. Notarized documents are not accepted. Please bring one (1) document from the list below for every name change from your Section 1 document to your current name:

  • Unexpired U.S. Passport
  • An official government-issued marriage certificate/license
  • Consular Report of Birth Abroad
  • Unexpired Permanent Resident Card
  • Naturalization/Citizenship Certificate
  • Unexpired Employment Authorization Card
  • Valid, unexpired foreign passport with a valid U.S. visa and valid I-94 issued by DHS. Other USCIS documents may be required depending on your status (DS-2019, I-20, etc.).
