This form should be used for claims concerning audits only when you believe a PBM has committed a violation to the Oklahoma Pharmacy Audit Integrity Act (PAIA). If you believe the Pharmacy Benefit Manager has committed a violation of the Patient’s Right to Pharmacy Choice, please submit the appropriate form.
