Transition Age Youth are individuals generally between the ages of 16 and 25 that are transitioning from child-serving systems to adult-serving systems. Youth transitioning from child welfare, residential care, and juvenile justice often experience challenges and barriers such as mental health needs, homelessness, and poor educational and vocational outcomes. These challenges and developmental stages for transition-age youth may be at risk for poor outcomes.
