Required Documents


Before You Begin

Depending on the service that you are looking to complete, you will be required to provide acceptable documents. Currently, there are three categories of documents: Primary ID, Proof of Oklahoma Residency, and 100% Disabled Veteran. Acceptable documents for each are listed below.

Use the Real ID checklist to see the required documents needed to add a REAL ID credential to your License or ID card.


Primary ID Documents

You will need to provide proof of identity/lawful presence in the U.S. The document must be an original or a certified copy. Notarized documents are not accepted. Examples of proof of identity include:

  • Valid, unexpired Oklahoma Driver License or State ID Card (not applicable to original REAL ID or original CDL issuance)
  • A Birth Certificate issued by the state in which you were born
  • Consular Report of Birth Abroad issued by the U.S. Department of State (Form FS-240, DS-1350 or FS-545)
  • Valid, unexpired U.S. Passport or Passport card
  • Valid, unexpired Permanent Resident Card issued by DHS or USCIS (Form I-551)
  • Naturalization/Citizenship Certificate issued by DHS (Form N-550, Form N-570, N-560, or N-56); Approval Certificates will not be accepted
  • Valid, unexpired employment authorization document (EAD) issued by DHS (Form I-766 or Form I-688B)
  • Valid, unexpired foreign passport with a valid U.S. visa and valid I-94 issued by DHS. Other USCIS documents may be required depending on your status (DS-2019, I-20, etc.)

Each document must be an original or a certified copy. Notarized documents, in addition to Pending or Approval applications, are not accepted.

As proof of identification and proof of lawful presence in the United States for a foreign national or legal permanent resident alien, you must provide one of the following:

 

  • Valid, unexpired I-551 Resident Alien/Permanent Resident Card
  • Valid, unexpired I-766 Employment Authorization Card
  • Valid, unexpired passport issued by a country other than the United States and I-94 card or Immigration (USCIS) Stamp
  • I-94, representing the following:
    • Temporary I-551
    • F-1 or F-2, with Form I-20
    • H-1A or B, H-2A or B, H-3, or H-4
    • J-1, with IAP-66 or DS-2019
    • J-2 (must be accompanied by and is a dependent of J-1 holder)

Legal Presence or Lawful Status Requirement
You will be asked to present your lawful presence documentation for each renewal or replacement service.

 

Note: The above does not contain the full list of approved documents.

If your name is different from the document which is presented from above (due to adoption, marriage, divorce, court-ordered name change), or is not the same on all your documents, you must provide additional documents with legal proof of every name change. If immigration documents are presented, the customer must provide proof of the updated name change with USCIS or SAVE.

Each document must be an original or a certified copy. Notarized documents are not accepted. Please provide one (1) document from the list below for every name change from your Section 1 document to your current name:

 

  • Valid, unexpired U.S. Passport
  • An official government-issued marriage certificate/license
  • Consular Report of Birth Abroad
  • Valid, unexpired Permanent Resident Card
  • Naturalization/Citizenship Certificate
  • Valid, unexpired Employment Authorization Card
  • Valid, unexpired foreign passport with a valid U.S. visa and valid I-94 issued by DHS. Other USCIS documents may be required depending on your status (DS-2019, I-20, etc.).
  • An official divorce decree
  • A court-ordered name change
  • Valid, unexpired REAL ID from another state 
  • Valid, unexpired U.S. Department of Defense ID, including IDs issued to dependents (military ID)

Social Security Number

For some services, you will be asked for your Social Security number. Your physical social security card is not required - it is only required that you know your number.


Proof of Oklahoma Residency Documents


  • Any correspondence delivered by the U.S. Postal Service, FedEx or UPS (Junk mail will not be accepted.)
  • Utility bill (electric, land or cell phone, gas/oil/propane, water, sewer, cable/satellite). The address being verified may be listed as a service address separate from mailing address.
  • OK state or tribal vehicle or boat title or registration
  • W-2 or any type of 1099
  • OK voter registration card
  • Statement from bank or other financial institution (savings or checking, mortgage, auto or other installment loan) Payday loan company documents are not accepted.
  • Copy of federal or OK tax return (Return must show manual or electronic signature or may show associated proof of electronic filing.)
  • USCIS form DS-2019, I-20 or I-797
  • Copy of lease signed by tenant and property owner
  • Homeowner’s, renter’s, life, health, or vehicle insurance policy or statement
  • OK Agriculture Exemption Permit (farm tax permit)
  • Pilot’s license
  • Social Security Administration correspondence
  • TSA HAZMAT or other TSA correspondence
  • Military correspondence
  • Oklahoma deed or land title
  • Public assistance correspondence
  • Correspondence from homeless shelter, halfway house or transitional service provider verifying that the provider receives mail for the customer
  • OK professional trade license issued by governmental agency
  • OK state, city, or county sales tax or business license
  • Property tax statement
  • OK handgun license
  • Copy of court order
  • Oklahoma school or education institution transcript or enrollment record

100% Disabled Veteran Documents

Each document must be an original or a certified copy. Notarized documents are not accepted.

  • A letter of proof from the Department of Veteran Affairs
  • A photo of your Sales Tax Exemption Card
