Required forms
- OMES Form CP001, Authorized Signature – Surplus Property Transfers have to be submitted and authorized ONLY by state agencies personnel, who is registered with the State Purchasing on the Authorized Signatures List. Surplus property transfer forms emailed from non-authorized agency personnel will be returned.
- OMES CAM Form SS-001A, Surplus Property Transfers (w/o Vehicles) – This is a state agency report of identified surplus property and an approval request to the Surplus Administrator for disposal of reported property except for vehicles. Instructions
- OMES CAM Form SS-001V, Surplus Property Transfers (vehicles only) – This is a state agency report of identified surplus vehicles and an approval request to the Fleet Manager for disposal of vehicles. Instructions
- OMES CAM Form SS-001T, Surplus Property Transfers (trailers only) – This is a state agency report of identified surplus trailers and an approval request to the Fleet Manager for disposal of trailers.
