Life Event

There are a few items you may want to update upon marriage. 

Insurance - Upon marriage, you are able to add your spouse to your insurance coverage.  Please complete the change request form and return to HR. You must supply documentation for all midyear insurance changes and they must be complete within 30 days of the event. 

W-4 - To update your tax withholdings, you will need to complete both the federal W-4 and the state withholding certificate

Beneficiaries - There are multiple beneficiaries you may wish to update upon marriage, including the wage beneficiary designation form.  Please contact HR for copies of the other forms. 

Direct deposit - You may wish to change where your paycheck is deposited. Please contact HR to obtain the form.  You will be required to provide a voided check or letter from your bank with the account and routing number. 

You can update your address in employee self service (link opens in new window) under self service>personal information>personal information.  Additionally, be sure to update your address in the Benefits Administration System (link opens in new window) under employee data>employee basic information.

Upon birth of your child, you can add your newborn to your insurance coverage. Please complete the change request form and return to HR.  You must supply documentation for all midyear insurance changes and they must be complete within 30 days of the event. 

Other qualifying events might include a change of dependents coverage, divorce, death, or other event that will cause the need for a change in coverage. If you are unsure, HR can assist you. For any change, you'll need to complete the change request form and return to HR. You must supply documentation for all midyear insurance changes and they must be complete within 30 days of the event. 

Certain illnesses and injuries qualify for protected leave called FMLA.  An employee is eligible for FMLA when they have been employed by the state for at least 12 months and have worked at least 1,250 hours during the 12-month period immediately before the date the FMLA leave is to start .  Please contact HR to clarify if you are eligible and receive applicable paperwork. 

You may also be eligible for FMLA if a qualifying family member is injured or ill.  Family members can include spouse, son, daughter or parent.  An employee is eligible for FMLA when they have been employed by the state for at least 12 months and have worked at least 1,250 hours during the 12-month period immediately before the date the FMLA leave is to start .  Please contact HR to clarify if you are eligible and receive applicable paperwork. 

Full-time OMES employees who have been with the agency for at least one year can apply for the Education Assistance Program.  This benefit will reimburse college tuition paid for qualifying courses as outlined by the policy.  Applications for the program must be submitted by March 1 for the classes that will be taken in the following fiscal year (July 1- June 30).  If you are interested in the program, please review the policy carefully to ensure you meet all requirements for reimbursement.

Work Event

If the work-related injury is serious or life threatening, the employee shall be transported to the nearest hospital or emergency room for immediate assistance.

If the work-related injury is not life threatening and the employee needs medical treatment, Human Resources shall contact the workers’ compensation provider for a list of physicians and facilities available to the employee.                                                       

Employees shall complete an incident report within one (1) working day of a reported accident or illness and submit to their supervisor for review. The supervisor will submit the incident report to Human Resources immediately following their review.

Additionally as an OMES employee, you can access LinkedIn Learning through your O365 account.  Please ask your supervisor about training opportunities specific to your team.

Telework and alternative work schedules are subject to supervisor approval.  Please review the policies and options below and speak with your supervisor about if one of these schedules will work for you.  Upon approval, please complete the relevant form and return to Human Resources.

Payroll Event

Please contact HR to obtain the direct deposit form.  You will be required to provide a voided check or letter from your bank with the account and routing number.

Retirement

If you are an OPERS member, your retirement eligibility is determined by age and service requirements. OPERS offers information tied to your specific eligibility factors based on when you started participating (link opens in new window).  Additionally, if you are within two years of retirement, you can attend a pre-retirement seminar (link opens in new window). 

If you are a Pathfinder member, you determine your path to retirement through savings.  After 5 years of service, you are fully vested.  However, official retirement age in the Pathfinder program is 65.  If you are 65, you will receive a prorated longevity check.  Please check out your plan highlights for more information, or contact your Retirement Plan Counselor (link opens in new window).
