HealthChoice High and High Alternative, Basic and Basic Alternative, and High Deductible Health Plan
HealthChoice is your plan of choice. There are five health plans available providing flexibility, savings and budget-friendly options. We are sure that one will meet the needs for you and your family.
All HealthChoice plans are PPO plans, which offers you the freedom to see any provider you choose by providing both network and non-network benefits. These plans offer several value-added benefits such as bariatric surgery, telemedicine and care management. The HealthChoice Select program is also included with all plans where select services and procedures can be covered at 100% of the bundled allowable fees with no out-of-pocket costs to members.

Medicare Supplement Plans
Plans for Medicare eligible former employees and dependents.
Welcome, State of Oklahoma retirees. We've got you covered with two Medicare plan options. Both include the benefits of original Medicare and Part D prescription drug coverage. You can see any doctor that accepts Medicare, and you never need a referral to see a specialist. You’re covered anywhere you travel in the U.S.
BlueSecure and Blue Cross Group MedicareRx (PDP) work together with original Medicare to lower your out-of-pocket costs and help cover your medical, hospital and prescription drug costs. It also covers skilled nursing facility care, Medicare Part A and B deductibles, and emergency care when you travel outside the United States. Blue Cross Group MedicareRx also covers many commonly used outpatient prescription drugs.
HealthChoice is proud to offer the HealthChoice SilverScript Medicare Supplement High Option and Low Option plans for our former and retired members. Both supplement plans provide the same medical benefits, allowing you to see any doctor throughout the U.S. that participates with Medicare. Medicare will continue to be your primary insurance.
HealthChoice will pay your Part A hospitalization deductible and you will be responsible for the Part B deductible. After the Part B deductible has been met, HealthChoice pays 20% of Medicare approved amounts. Both plans include Medicare Part D prescription drug coverage, keeping all coverage in one place for members. The prescription drug benefit differs between the two plans. BCBSOK-MAPD is a Medicare Advantage plan that bundles prescription drug coverage and supplemental benefits with original Medicare. It’s your one-stop shop for all your health insurance needs. It covers most commonly used services such as provider visits, inpatient and outpatient hospital services, emergency care and prescription medicines.
Your Medicare paperwork, claims and benefits are managed so you have only one convenient call to make when you have questions.
CommunityCare’s Senior Health Plan (HMO) members are served by Oklahoma’s largest, locally owned health insurance company.
CommunityCare is owned by Saint Francis Health System and Ascension St. John – and we have the only Medicare Advantage plans contracted with these two premier health systems. Additionally, Senior Health Plan has the highest customer satisfaction rating for Medicare plans in Oklahoma!

BlueCare Dental offers two affordable plans to meet your budget – both have access to the same dental network across the entire country, so you can always find a dentist no matter where you are. Our plans provide coverage for preventive services like checkups, cleanings and basic X-rays at zero cost to you; something else to smile about!
The Cigna prepaid dental plans are simple, predictable and affordable. All covered dental services are listed in your plan’s specific Patient Charge Schedule, so you can always count on predictable coverage and affordable out-of-pocket costs at the dentist (must be in-network) when planning your and your family’s dental care.

Delta Dental of Oklahoma is the state’s leading dental benefits provider, offering the largest network of dentists and a 99% member satisfaction rating. Two quality dental benefit plan options are available to state employees.
Delta Dental PPO covers preventive care at 100% and provides access to both the Delta Dental PPO and the Delta Dental Premier networks. Delta Dental PPO – Choice is a low-cost program that gives eligible members access to the Delta Dental PPO network. Costs are determined by a set fee table which outlines the copayment for each covered service.
Subscribers are welcome to receive treatment from the licensed dentist of their choice, but will enjoy lower out-of-pocket expenses when visiting a Delta Dental PPO participating dentist.
At HealthChoice, we love to make you smile. HealthChoice offers members a convenient and easy-to-use full-service dental plan. Members can use any dentist of their choice but will see a greater benefit by using a network provider.
HealthChoice covers preventive services at 100%, including routine oral exams and cleanings. Basic services such as fillings, extractions and oral surgery are covered at 85%, while major services like implants, partials and dentures are covered at 60%. These services have a yearly maximum benefit of $2,500/person. Orthodontia is available for children under the age of 19 that have been on the HealthChoice plan for 12 consecutive months.
