Applications for Disaster Unemployment Assistance (DUA) is available for those who were impacted by the severe weather that occurred on April 25, 2024 through May 9, 2024 in Carter, Hughes, Love, Murray, Okmulgee, Pontotoc, Osage and Washita counties. The deadline to submit a claim for DUA 4776 is July 1, 2024. Applications submitted after that date will be considered untimely unless the individual provides good cause for filing after the deadline. Open or public records specifically associated with the Oklahoma Employment Security Commission may be requested pursuant to the Oklahoma Open Records Act. This act is a series of laws designed to guarantee that the public has access to public records of governmental bodies in Oklahoma. To make a request, download and submit the form below.

To obtain records specific to an unemployment claimant or related employer, please follow the instructions in the section below.

Request Closed / Confidential Records

There are two ways to obtain closed or confidential records from OESC.  

  1. Obtain a court order signed by the judge.  This can usually be obtained by motion to the court with notice to all adverse parties.  It is not necessary to include OESC as a party in interest or give notice to OESC of any hearing dates.  Once the order is obtained, OESC will begin processing the request.
  2. Use a Waiver of Confidentiality. Depending on the need, two waiver forms are available.

    1. Individual Waiver of Confidentiality - For individual records, the individual that the records relate to must sign the form waiving their confidentiality.
    2. Employer Waiver of Confidentiality - Employer records must be signed by a corporate officer with authority to bind the employer. The signature must be notarized and OESC must receive the original, notarized document. 

All requests for individual records must contain the Social Security Number of the individual that records are being requested on.  Request for employer records must contain the employer's UI Tax Account Number.  Both of these requirements are included in the Waiver of Confidentiality forms.  All Waivers must be signed by a notary and the original submitted to OESC's Custodian of Records.  All court order requests should include a cover letter with the required information (SSN/Account Number), because court orders will not publish this information.

There is a $10.00 search fee, plus $0.25 for each page copied on each record request.  If certified copy of records are requested, there is a $1.00 certification fee.  An invoice will be sent once the documents are retrieved.
