- If Study Participants met certain eligibility requirements, savings in SEED OK was matched up to $250 per year during 2008-2011. The Match Eligibility Form allowed the state to check records and see if Study Participants were eligible for the Savings Match. The Match Eligibility Form also authorized the release of certain information.
- Along with the Match Eligibility Form, Study Participants who did not file a 2009 Oklahoma State Income Tax Return and did not receive certain benefits from the Oklahoma Department of Human Services during 2009 could have completed the Self-Certification Form to determine eligibility for the Savings Match.
