How To Obtain an NPI Number


 The NPI is issued through the National Plan and Provider Enumeration System (NPPES) on behalf of the Centers for Medicare & Medicaid Services (CMS). 

✅ You Need a Type 1 NPI (Individual Provider)

As a chiropractor, you will apply as:

  • Type 1 – Individual Healthcare Provider

Even if you own a practice, your personal provider number is Type 1.
(If you form an LLC or corporation, you may also need a Type 2 for the business entity.)

1️⃣ Create an I&A Account

  • Go to: https://nppes.cms.hhs.gov
  • Click Create or Manage an Account
  • Set up an Identity & Access (I&A) account if you do not already have one. 

2️⃣ Start the Application

  • Log in

  • Select Apply for an NPI

  • Choose Type 1 (Individual)


3️⃣ Information You’ll Need Ready

  • Full legal name (exactly as on license)

  • Social Security Number (for verification only)

  • Date and place of birth

  • Practice location address

  • Mailing address (if different)

  • State chiropractic license number

  • License state

  • Primary taxonomy code

For chiropractors, the primary taxonomy code is typically:

  • 111N00000X – Chiropractor

4️⃣ Submit the Application

  • Review carefully

  • Submit electronically

  • You’ll receive confirmation by email

Processing time is typically 1–10 business days.

📌 Important Reminders

  • There is no fee to apply.

  • Your NPI does not expire.

  • You must update changes (address, license, etc.) within 30 days.

  • Keep your login information — you’ll use it to update records in the future.
