Definitions and Guidance
- Proof of Publication:
- An affidavit from the newspaper used to publish your notice of intent to apply for an alcoholic beverage license. It requires a copy of the article and the two dates it ran in the newspaper. If your newspaper does not provide its own affidavit, they can complete our Proof of Publication.
- Individual History Forms:
- A form that must be completed by ALL applicants: individuals, partners, corporate officers, directors, stockholders, LLC managers, LLC members, tribal members, trustees, and government entity staff. This must be completed and uploaded when submitting the online licensing application.
- Location Diagram:
- A diagram that includes all areas: outside dimensions, rooms, doorways, bars, liquor storage areas, patios, pool areas, etc. The Location Diagram must be completed and uploaded when submitting the online licensing application. It is required for all new business applications that require a physical location inspection. Note: Blueprints are NOT accepted.
- Self-Distribution Affidavit:
- Required for certain licenses, specifically the Small Brewer Self-Distribution and Winemaker Self-Distribution application. Attestation of eligibility is required to complete the application.
- Certificate of Liability Insurance:
- Proof of Liability Insurance covering bodily injury and property damage must be maintained by an ABLE licensee as long as an ABLE license is active.
- Certificate of Incorporation:
- Provided to a corporation by the Secretary of State’s office when a corporation is registered. If you do not have a copy, contact the Secretary of State’s office from the State the corporation was registered in. If you have formed a corporation in Oklahoma, contact the Oklahoma Secretary of State. Oklahoma Secretary of State - HomeCertificate of Good Standing: Provided to an entity by the Oklahoma Secretary of State stating the corporation, LLC, LP, LLP, etc., is in good standing once a business entity is registered in Oklahoma.
- Certificate of Authority or Registration:
- Provided to a business entity by the Oklahoma Secretary of State stating the corporation, LLC, LP, LLP, etc., is registered and authorized to do business in Oklahoma if the business was created in another state.
- Election Minutes:
- A written copy of a meeting held to elect and list the officers, directors, and stockholders of a corporation or non-profit organization or non-profit corporation.
- Articles of Organization:
- A document filed with the Oklahoma Secretary of State’s office on behalf of an LLC. A filing number and/or a certificate of limited liability may be issued. For an LLC formed outside of Oklahoma, the articles may be called a “Certificate of Formation.”
- LLC Operating Agreement:
- An agreement that outlines who the manager/members are and the functional decisions, rules, and regulations of the LLC including a schedule or attachment showing the membership interest.
- Deed, Lease, Management Agreement, or Sales Contract:
- A document that shows proof of the legal right to conduct business at a particular location. The applicant entity listed on the ABLE application must be the same entity that is listed as
- The grantee on a deed
- The tenant/lessee on a lease
- The manager on a management agreement
- The buyer on a sales contract.

- Certificate of Compliance:
- Provided by the City Clerk’s office, if the business is within the city limits, or the Board of County Commissioners if the business is located outside the city limits. The letter or certificate must state that the business location meets the zoning, fire, safety, and health codes of the area. If no codes exist in the city or county of the applicant location, a letter or certificate must be provided stating no zoning, fire, safety, and health codes exist. If the county the business is in defers all inspections to the state, copies of the inspections made by the State Fire Marshall’s office and the State Health Department must be provided. NOTE: This letter cannot be over sixty days old at the time of application.
- Tax Statement:
- A letter or receipt prepared by the County Treasurer’s office from the County where the business is located. This letter or receipt must state no real or personal property taxes are owed, or the applicant's name is not on their tax rolls for previous years.
- Tax Exempt 501c Letter:
- A letter from the IRS stating the organization is tax exempt.
