Dec. 19, 2025, Incident: Oklahoma Tax Commission

On Dec. 19, 2025, the Oklahoma Tax Commission (OTC) identified a data security incident that resulted in unauthorized access to tax information held within the Oklahoma Taxpayer Access Point (OkTAP) system. It is not clear how many individuals were impacted by the breach. Following the incident, OTC has implemented several additional security measures to prevent this event from occurring in the future.

Impacted individuals have been notified directly by mail with details regarding the event and information about complimentary credit monitoring and fraud assistance services. Impacted individuals may call the dedicated assistance line at 833-319-9482, Monday through Friday, from 7 a.m. to 7 p.m. Central time (excluding major U.S. holidays).

OTC is providing these services to affected individuals for 12 months.
