HealthChoice High and High Alternative, Basic and Basic Alternative, and High Deductible Health Plan
HealthChoice is your plan of choice. There are five health plans available providing flexibility, savings and budget-friendly options. We are sure that one will meet the needs for you and your family.
All HealthChoice plans are PPO plans, which offers you the freedom to see any provider you choose by providing both network and non-network benefits. These plans offer several value-added benefits such as bariatric surgery, telemedicine and care management. The HealthChoice Select program is also included with all plans where select services and procedures can be covered at 100% of the bundled allowable fees with no out-of-pocket costs to members.
