Application/Licensure Record Copies
The Board no longer has a form to request Board records. You may request copies of Board records by emailing the Board at info.behavioralhealth@bbhl.ok.gov
Please include you name, license type, license number (if available) and the type of record you are requesting.
There is a fee of .25 cents per page, a $25.00 per hour administrative fee, and the cost of shipping.
All cost must be paid in full before the requested records(s) are mailed.
