Application/Licensure Record Copies
The Board no longer has a form to request copies of your application and/or licensure records. You may request copies of your application and/or licensure records by emailing the Board at info.behavioralhealth@bbhl.ok.gov
Please include you name, license type, license number (if available) and the type of record you are requesting.
If the record you are requesting is less than 20MB we will attempt to return the record copy to you electronically. There is no cost associated with an electronic response. If we are unable to respond by email, there is a fee of .25 cents per page a $25.00 per hour administrative fee, and the cost of shipping.
All cost must be paid in full before the requested records(s) are mailed.
