2024 Oklahoma State Board Exams

Licensure in Oklahoma is obtained by the passage of the Oklahoma State Board Exams. To be a candidate for the Oklahoma State Board Exams the following requirements must be met:
- You must be twenty-one (21) years of age
- You must be of good moral character
- You must have met the undergraduate requirements and are a graduate of an accredited school of optometry, conferring the degree of Doctor of Optometry
- You must have passed the NBEO Parts I, II, and TMOD Exam. (If you did not pass the NBEO TMOD Exam, you will be required to pass the Oklahoma State Board TMOD Exam on July 20, 2024)
- You must have passed the Laser Therapy for the Anterior Segment Course offered by Northeastern State University as a prerequisite for taking the Oklahoma Boards
Requirements
We now require that each applicant for licensure (NOT license renewal) provide fingerprint images to be used for the purpose of permitting a state and national criminal history records search through the OSBI and FBI. We will mail you the fingerprint cards and instructions once you go online and apply. We must have your completed fingerprint cards in our office no later than June 13, 2025.
The Oklahoma State Board Exams are given once annually. The 2024 Oklahoma State Board Exams are set for June 13, 2025 and will be offered at the Northeastern State University Oklahoma College of Optometry. There is a $200.00 non-refundable examination fee which MUST be paid online. We must have the following documents in our office by Friday, June 13, 2025:
- Your original Pre-Optometry school transcripts (unopened or emailed directly from the school to jessica.cobble@optometry.ok.gov)
- Your "Final" Optometry school transcripts (unopened or emailed directly from the school to jessica.cobble@optometry.ok.gov)
(This transcript must contain your Doctor of Optometry Degree) - A current 2"X 3" wallet sized professional photograph, no copies accepted
- Letter of Good Standing from any state in which you are licensed or have been licensed
- Work History (Resume or CV)
- A current cardiopulmonary resuscitation certificate
- Your National Board Scores Part I and II including the TMOD Exam sent directly from the NBEO
Our Online License Registration System will be available May 1, 2025!
Beginning May 1, 2025 you may use the License Application System to register for the 2025 Oklahoma State Board Examination.
