Oklahoma Board of Examiners in Optometry
The non-refundable examination fee is $200.
Please note: Your ORIGINAL transcripts for your Pre-optometric and Optometric Education are needed to complete your application as well as a copy of your current CPR card.
Application for Second Office
You may register your second practice location with your regular license renewal using the same License, Application, and Renewal System:

The registration fee is $100.
