If you have a complaint against an optometrist, you may file a formal complaint with the Board of Examiners in Optometry by filling out the form below and mailing or emailing (optboard@optometry.ok.gov) it to our office. Or you may use the complaint portal located at Oklahoma Board of Examiners in Optometry. If you file a complaint online, please download and complete the Release of Records below and attach it to the complaint or email it to our office at optboard@optometry.ok.gov.
