Adult Day Centers can provide family caregivers with a needed daytime break while giving their loved one who is aging or an adult with special needs a chance to socialize with their peers.
Help Returning Home
The Living Choice program is designed to transition individuals with disabilities and long-term illnesses from institutions back into their homes in the community.
State Plan Personal Care (SPPC)
Assists people of all ages who qualify financially and medically with basic needs such as bathing, toileting, meals, etc. to remain at home.
