Nursing Facility services are those services furnished pursuant to a physician's orders which care requires the skill of technical or professional personnel.

Affordable Assisted Living
Assisted Living is a service option available to eligible participants through the Home and Community Based ADvantage Waiver program.

Residents’ Rights (Ombudsman)
The Ombudsman Program serves residents in Oklahoma’s long-term care facilities, including nursing homes, assisted living and similar adult care homes. An Ombudsman helps improve the quality of life and quality of care available to long-term care facility residents.

Medicaid ADvantage Program
The Home and Community Based ADvantage Waiver program provides alternatives to placement in a nursing facility for eligible adults. 

Adult Day Centers 
Adult Day Centers can provide family caregivers with a needed daytime break while giving their loved one who is aging or an adult with special needs a chance to socialize with their peers.

Help Returning Home
The Living Choice program is designed to transition individuals with disabilities and long-term illnesses from institutions back into their homes in the community.

State Plan Personal Care (SPPC)
Assists people of all ages who qualify financially and medically with basic needs such as bathing, toileting, meals, etc. to remain at home.
