- Official State or Government-issued Driver's License or ID
- Alien Registration Number
- Government Passport Number
- Employer or taxpayer identification number
- Educational Records
- Employment Information
- Malpractice Insurance Information
- Discipline Records
Information shared with:
- Oklahoma State Department of Health
- United States Drug Enforcement Administration
- Oklahoma Bureau of Narcotics
- Oklahoma Dental Association
- Oklahoma Dental Hygiene Association
- Oklahoma Health Care Authority
- National Practitioner Data Bank
- Individual Information may be shared with official State and Federal agencies on a case-by-case basis or pursuant to a valid subpoena.
All personally identifiable data is stored on secure OMES servers, maintained by OMES. (Office of Management Enterprise Systems)
