NOTICES

The Board office will be closed for business July 27-31, 2026 for building maintenance.  Please make sure you plan accordingly for anything you may need from us that month.  We will not be processing mail and will process emails once we return to the office.  ALL Dentist and Dental Hygienist applications for the August 21st Board Meeting will need to be in the office no later than July 24th, 2026 (REQUIRED DOCUMENTS: Along with these applications, they MUST have the National Practitioners Data Bank and any state verification of licensures to be considered for the agenda). 

 

*ALL DENTAL ASSISTANTS MUST HAVE A PERMIT BEFORE WORKING CHAIRSIDE AS OF NOVEMBER 1, 2025.*

 

 

*** As of 3/25/26 we were notified that the SCAM callers are now using Board Staff names.  Please notify the board by email to obod.board@ok.gov when you receive these calls with any details you are able to give.  Also, board staff will not be pushy or request money.  ***

Dentistry Data Policy

The board office does NOT have access to usernames or password and we cannot reset them!

  • Social Security Number
  • Official State or Government-issued Driver's License or ID
  • Alien Registration Number
  • Government Passport Number
  • Employer or taxpayer identification number
  • Educational Records 
  • Employment Information
  • Malpractice Insurance Information
  • Discipline Records

Information shared with:

  • Oklahoma State Department of Health
  • United States Drug Enforcement Administration
  • Oklahoma Bureau of Narcotics
  • Oklahoma Dental Association
  • Oklahoma Dental Hygiene Association
  • Oklahoma Health Care Authority
  • National Practitioner Data Bank
  • Individual Information may be shared with official State and Federal agencies on a case-by-case basis or pursuant to a valid subpoena.
