The Board office will be open Wednesday, November 27, 2024 for walk-ins with minimal staff.  Any questions please email the Board email at obod.board@ok.gov.  The office will be closed for the Thanksgiving holiday, Thursday and Friday November 28-29, 2024.  We will resume normal business, Monday, December 2nd.

General Questions


The best way to contact the board is by email.  For Dentists, Hygienists, Dispensing, or Corporations please contact ashley.adkins@dentistry.ok.gov.  For Dental Assistants, OMSDA's, and Dental Labs please contact naukeshia.johnson@dentistry.ok.gov.  If you need the board directly, email obod.board@ok.gov.

If you cannot find your name on the website, you are more than likely not in good standing with the Board Office. Please call the office at (405)522-4844 for further information.

Dentists, we are updating the database, please contact Ashley Adkins, if you find your name is not showing on the list and it should be.

Anyone may request an address list for a fee of $75 per list request.  Fill out the request form, submit the fee by check or money order and have an email address to send the excel file to.  Please make sure your writing is legible on the request form.    

Yes. State statute requires you to provide a complete copy of the patient's file upon request. You may charge for the reproduction of the file. The fees for reproduction are set by the same statute - 76, OS, sections 19 & 20. Patient files may not be held hostage pending settlement of an unpaid balance.

No. With a law change effective Nov. 1, 2012, the grace period has been eliminated. All renewals and payments are due to the Board office by December 31st of each year. You may still renew until March 31st by paying the renewal fee plus late fee, but your license or permit is considered EXPIRED and it would be a violation of the State Dental Act to work on an expired license or permit.

You will need to mail in a written request with a $10 check or money order. Please use the Name Change/Copy Request form. This form will also be used for name changes; please submit the form and marriage license, divorce decree, or court order with the request and payment.

Verification of licensure or Letter of Good Standing; please use the attached form and fee of $10 by check or money order for each request. 

You will need to login to your online account and change your information there. We will then get notification the following morning and update our records.

If you have not received an email from the Board in regards to your application within 3 weeks of submitting it, please call the office at (405)522-4844.

Always check the Board of Dentistry’s website. Anytime any big change or situation happens, we will post it to our website. We also have a Facebook page, and plan on starting to send out email notifications in the very near future.

Anyone who performs ANY duties as an assistant in any way must be permitted.

You will need to go to our website at Oklahoma.gov/dentistry. Under the “Applications and Forms” tab, you will find the link you need depending on if you are a dentist, hygienist, or dental assistant. Each link will provide you with the list of things you need to submit to the Board.

Although it is not law that an X-Ray must be taken every year, the definition of "patient" or "patient of record" means an individual who has given a medical history and has been examined and accepted by a dentist for dental care. Therefore, it is dentist discretion on what he/she considers an exam and the Board of Dentistry cannot tell a dentist how to run his/her practice.
