**The Board office will have limited staff on Friday, October 2nd due to the Study Session in Durant.  There will be a staff member to accept applications/payments and answer phones.**

 

 

*** As of 3/25/26 we were notified that the SCAM callers are now using Board Staff names.  Please notify the board by email to obod.board@ok.gov when you receive these calls with any details you are able to give.  Also, board staff will not be pushy or request money.  ***

Renewals

Print a renewal form and mail it along with the listed fee must be check or money order,NO CASH WILL BE ACCEPTED and fees are non-refundable.  The address is listed on at the top of the form and make checks payable to OKBOD.

Call the Helpdesk at 405-521-2444 for help logging into the License/Permit site.

Renewal Forms

Just a note for Hygienists- all advanced procedures are a ONE time fee, please do not add an additional $10 to your check or money order when you are renewing by paper form and already have the advanced procedure on your license.
