Please note that board staff is diligently working to get all renewals mailed out as quickly as we can.  We are approximately a week behind on renewals.

Renewals

Print a renewal form and mail it along with the listed fee must be check or money order NO CASH please.  The address is listed on at the top of the form and make checks payable to OKBOD.

Call the Helpdesk at 405-521-2444 for help logging into the License/Permit site.

Renewal Forms

Just a note for Hygienists- all advanced procedures are a ONE time fee, please do not add an additional $10 to your check or money order when you are renewing by paper form and already have the advanced procedure on your license.
