****Hygienists and Dental Assistants- we attempted to upload all of the Advanced Procedures and Expanded Functions to your online accounts.  The report some how added some to the online accounts that are incorrect, we are working with our IT department to correct them.  Please be patient and rely on the verifcation list and not what is shown on your online accounts.****

Renewals

Print a renewal form and mail it along with the listed fee must be check or money order, NO CASH WILL BE ACCEPTED and fees are non-refundable.  The address is listed on at the top of the form and make checks payable to OKBOD.

Call the Helpdesk at 405-521-2444 for help logging into the License/Permit site.

Renewal Forms

Just a note for Hygienists- all advanced procedures are a ONE time fee, please do not add an additional $10 to your check or money order when you are renewing by paper form and already have the advanced procedure on your license.
