License Verification
If you would like to obtain a Verification of Licensure from us, please complete Verification Request form. You will need to print this form and mail it with a $10 check or money order. Verifications cannot be requested by telephone.
You may request we send it directly to the state you are applying in, or you can request we send it back to you. Please ensure to read the instructions for the state you are applying for, so you know how they want you to provide that information.

If you cannot use the above links for verification, or if you are looking for an inactive licensee or permit holder that is not on this list, you must mail in the Verification Request form with a $10 check or money order to obtain a verification. NO CASH WILL BE ACCEPTED.
Open Records Requests
Board Orders for a licensee that has disciplinary action(s) will be found once you click the verification list you need, then click "View the Disciplinary Licensee" link or if you have any other questions regarding this information, please fill out an open records request form and email your request to the Deputy Director, Jeff Puckett.
